About Us

Welcome to Penwith PCN!

This is an exciting time in the PCN. Innovation, creative problem-solving and strong links to our local communities are the bedrock of our PCN and we want to build on that to reach more people, in new ways.

We are a network of seven GP practices across St Ives, Hayle, Penzance, Marazion and St Just. Our registered patient population is just over 67,000 people and that includes people in the highest levels of deprivation in the county.

Penwith is a beautiful place to live, but at the same time there are many local challenges that can affect health and wellbeing.

We seek to understand ‘what matters to you’, not ‘what’s the matter with you’. We are a diverse team of people who are out-of-the-box thinkers, with a wide range of skills, who aim to provide truly personalised care. What matters to us is local, responsive, compassionate care, close to home.

What is a Primary Care Network?

Primary Care Networks were introduced in 2019 as part of the NHS Long Term plan.  As the population grows and people live longer, often with more complex needs,  GP services have had to adapt to these changing needs.

GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in local areas in groups of practices known as Primary Care Networks.

Additional Roles Reimbursement Scheme (ARRS)

Primary Care Networks have been given funding to provide additional roles to create bespoke multi-disciplinary teams within their individual network. 

PCN’s assess the needs of their local population and, working with local community services, make support available to people where it is most needed.

Ellie

Team Lead
Ellie shares her working week between. Penzance, Hayle, St Ives and Pendeen. Ellie has worked in the social sector from multiple angles including schools, prisons, care homes, homeless hostels, addiction recovery centres and out in the community. Ellie performance managed one of the most successful ESF welfare to work programmes in Europe,  set up structured day programme in a homeless hostel (with no funding!) Ellies models of working have been published in a life coaching magazine on a few occasions. Her experience is backed up by qualifications in psychotherapy, hypnotherapy, NLP, health and community studies, and more.

Free

Shares her working week between Penzance, Hayle and St Ives and has been with us since the service first started in 2023. Before stepping into this position, Free worked throughout several areas within the local mental health services, starting in the crisis service at Longreach House where she supported people across the acute mental health wards. From there, she supported individuals who used the Mental Health Connect Line and later working within the West Home Treatment Team. Across these roles, Free worked closely with people facing long-term mental illness and crisis, supporting them to access recovery pathways and working to prevent hospital admissions. Free has a particular interest in the connections between mental health and nutrition, and how small changes can help people feel more balanced and in control. This interest led her to study a foundation degree in Health & Nutrition, followed by a degree in Psychology. Free is passionate about confidence-building through coaching, and will often draw on her own lived experiences to offer meaningful, compassionate support.

Joanna

Joanna has been with us since May 2024 and her approach is grounded in compassion, trauma awareness and a deep belief in everyone’s capacity to heal and grow with the right support. Joanna brings a rich background of working within specialist services and grassroots charities supporting people who are facing discrimination and abuse, often with complex mental health needs. Joanna has a particular interest in working with grief and in her own time, voluntarily holds a monthly grief circle in her community. Joanna offers a safe space for those who need it, to share and be witnessed in their grief, whatever form that takes. Joanna feels passionately about working together within nature to bring a sense of grounding, community and belonging.

Sandra

Sandra shares her working week between Hayle and Penzance. Sandra joined the team in August 2024 and has a background centred around wellbeing, as a previous massage therapist, a nutritional therapist, a life coach, and a yoga teacher. Sandra loves that her role allows her to bring all of her knowledge and experience to provide a person-centred approach to whoever she sees.

Matt

Matt is the newest member of our team, joining from a career as an NHS doctor, having worked across a range of specialties supporting people through complex physical and mental health challenges. He has a particular interest in the connection between mind and body and aims to offer a calm, grounding space where people feel heard, understood and supported to make meaningful, achievable changes in their mental wellbeing.

Sara supports patients to access community groups, services and organisations, helping them improve their health and wellbeing in a non-clinical way. She has previously worked in primary and secondary education and extensively across the NHS, including cardiac physiotherapy, community rehabilitation and discharge, and the integrated community stroke service, bringing a wealth of understanding and experience about the importance of healthy aging and engaging in social connections for health and wellbeing.

Supporting patients to improve their health and wellbeing by connecting them with community groups, activities, and local services that matter to them. With a background as a local councillor and former Fundraising Manager for a national charity, Klara is passionate about building strong, supportive communities and helping people navigate life’s challenges. Klara believes that small connections can make a big difference, and she is here to help you find the right support, at the right time.

Tim

Tim was born and raised in the Penzance area before going to University in London and then Cardiff. He has worked as a Physio since 2007 in various roles including outpatients, orthopaedics, hand therapy and community rehab. Since 2001, Tim has been sharing his working week between Morrab and Rosmellyn GP Practices. Tim has two boys and in his spare time, enjoys playing tennis, surfing and sea swimming.

Gemma

Gemma has worked across a wide range of specialties before specialising in Musculoskeletal Physiotherapy in 2011 including working at the Physiotherapy Department at  West Cornwall Hospital from 2011 until 2021 where she became the First Contact Physiotherapist. Gemma has also worked in the Paediatric Orthopaedic Physiotherapy Team at RCHT for 8 years. Gemma shares her working week between Marazion Surgery and Sunnyside Surgery
